    This study investigates different ways of efficiently educating learners to become bi- or multilingual. In the introduction relevant background information concerning the definition of bilingualism is given and related issues are being discussed, such as language learning, language awareness, bilingual instruction and bilingual education models. Human beings are characterised by language, but also by social and cultural tradition. In the context of globalisation pluralism and multiculturalism are common factors affecting even previously isolated communities. Bi- and multilingualism are some of the strategies of adaptation to this reality. It can be assumed, that this development will continue rapidly as the inhabitants of the global village grow in and adapt to this realisation. In countries with a history and tradition of multilingual and bilingual education like South Africa, Canada, Luxembourg and Belgium and also in countries where immigration results in issues of multilingual and bilingual challenging society like in the USA one finds a great variety of answers to these problems posed. The situation in Germany is different, because bilingualism became an issue only fairly recently due to the actual demands brought by the European union. In South Africa the social context determines largely that people are inclined to bi- and multilingualism and the educational system tries to promote this predisposition by means of bi- or multilingual models in schools, so that the learners become efficient facing the bi- and multilingual challenges of their diverse and pluralistic society. In the USA on the other hand the need arises to provide for minority groups, so that they can retain their mother tongue (e.g. Spanish) while becoming fluent in the predominant language (i.e. American). Lastly the new development of the European Union has led Germany to actively promote bi- und multilingual education to aid the integration of Germans in the Union. The various models developed in these countries are studied in the wide variety of their different social and historical contexts. The main part of the study comprises a systematic overview of the various bilingual education models that are being implemented in different countries. These countries include the USA, Canada, Germany, Luxembourg, Belgium and lastly and more detailed, South Africa. A number of different circumstances surrounding bi- or multilingualism in the various countries have given rise to a large variety of models attempting to address the challenges posed by the various issues. A number of different goals have been addressed and the results have been just as varied. This leads to a concluding comparison of the western and European models with those of the South African models. Much research has been done on bi- and multilingualism and specifically bilingual education, as humans since earliest times were concerned with this subject. For my study I have relied mainly on more recent sources (1990 to the present) from the USA, Canada and Germany. However as the academic study of this issue has been going on for quite a time in some countries like South Africa, I have also referred to older books and journals (1945<).     Utilization of dental services and health literacy by older seniors during the COVID-19 pandemic

    Background: This study aimed to investigate the utilization of dental services by older seniors during the COVID-19 pandemic and to evaluate their ability of finding, understanding, and using information on COVID-19. Methods: At the end of February 2021, a survey addressing demographic characteristics of the participants, (pain-associated) utilization of dental services, worries regarding a potential COVID-19 infection, the individual use of protective masks, and difficulties regarding the access to information on COVID-19 (by using the modified European Health Literacy Questionnaire [HLS-EU-Q16]) was developed. It was sent to all patients of the Dental Clinic of University of Leipzig who were either 75, 80, or 85 years old (n = 1228). Participation was voluntarily and anonymously; questionnaires had to be returned within six weeks, no reminders were sent. Results: Of the 439 replies (response rate 35.7%), twelve were excluded from data extraction due to disinterest, dementia, or lack of age information. Of the older seniors, 81.5% (n = 348) had utilized at least one dental examination and 54.2% of the dentulous patients (n = 199) had attended at least one dental hygiene appointment within the past year. Up to 55.8% of all participants said it was "difficult" or "very difficult" to find, understand, and use information on COVID-19, especially when judging reliability of information presented in the media, which was especially true for seniors with assigned care levels presenting odds ratios up to 5.30. Conclusions: The investigation revealed a frequent utilization of dental services by older seniors during the COVID-19 pandemic. However, the older seniors encountered difficulties finding, using, and understanding information about COVID-19.     Complicated Postoperative Course after Pulmonary Artery Sling Repair and Slide Tracheoplasty.

    Pulmonary artery sling (PAS) is a rare congenital condition in which the left pulmonary artery (LPA) arises from the right pulmonary artery, and then passes between the trachea and the esophagus to reach the left lung, thereby forming a sling around the airway. It is often associated with intrinsic tracheal stenosis due to complete cartilaginous rings. Therapeutic management nowadays consists of one-stage reimplantation of the LPA and tracheoplasty with cardiopulmonary bypass support. Here, we present a 7-week-old boy with PAS and long-segment tracheal stenosis (LSTS) who underwent surgical intervention consisting of reimplantation of the LPA and slide tracheoplasty. Multiple respiratory and cardiovascular complications marked the postoperative course. They consisted of recurrent failed attempts in weaning off mechanical ventilation due to bronchomalacia, left vocal cord paralysis, development of granulation tissue at the anastomosis and restenosis of the trachea, and the main stem bronchi requiring balloon dilatation. The patient also developed bilateral pulmonary artery thrombosis and stenosis of the LPA. After a prolonged hospitalization, the patient is doing well without any respiratory symptoms and has a good result on follow-up bronchoscopy 1 year after the initial surgery. The stenosis of the LPA responded well to percutaneous balloon dilatation 12 months after the primary surgery. The case illustrates that even though surgical techniques are improving and are in general associated with a low morbidity and mortality, management of PAS and tracheal stenosis can still be challenging. However, good long-term outcome can be achieved if the initial postoperative phase is overcome

    Hemipiperazines as peptide-derived molecular photoswitches with low-nanomolar cytotoxicity

    The development of photochromic systems is an important and growing area of research, in particular for bioactive molecular photoswitches. Here, the authors report on photopharmacological antimitotic agents, operational under visible light, based on a peptide-derived hemipiperazine photochrome. Molecular photoswitches transform light energy into reversible structural changes. Their combination with known pharmacophores often allows for photomodulation of the biological activity. The effort to apply such compounds in photopharmacology as light-activated pro-drugs is, however, hampered by serious activity reduction upon pharmacophore modifications, or limited biostability. Here we report that a potent antimitotic agent plinabulin and its derivatives demonstrate up to 56-fold reversible activity photomodulation. Alternatively, irreversible photoactivation with cyan light can enhance the cytotoxicity up to three orders of magnitude-all without compromising the original activity level, as the original pharmacophore structure is unchanged. This occurs due to the presence of a peptide-derived photoswitchable motif hemipiperazine inside the plinabulin scaffold. Furthermore, we systematically describe photochromism of these thermally stable and biocompatible hemipiperazines, as well as a photoswitchable fluorophore derived from plinabulin. The latter may further expand the applicability of hemipiperazine photochromism towards super-resolution microscopy.Peer reviewe

    A systematic review of vitamin D status in populations worldwide

    Vitamin D deficiency is associated with osteoporosis and is thought to increase the risk of cancer and CVD. Despite these numerous potential health effects, data on vitamin D status at the population level and within key subgroups are limited. The aims of the present study were to examine patterns of 25-hydroxyvitamin D (25(OH)D) levels worldwide and to assess differences by age, sex and region. In a systematic literature review using the Medline and EMBASE databases, we identified 195 studies conducted in forty-four countries involving more than 168000 participants. Mean population-level 25(OH)D values varied considerably across the studies (range 4·9-136·2nmol/l), with 37·3% of the studies reporting mean values below 50nmol/l. The highest 25(OH)D values were observed in North America. Although age-related differences were observed in the Asia/Pacific and Middle East/Africa regions, they were not observed elsewhere and sex-related differences were not observed in any region. Substantial heterogeneity between the studies precluded drawing conclusions on overall vitamin D status at the population level. Exploratory analyses, however, suggested that newborns and institutionalised elderly from several regions worldwide appeared to be at a generally higher risk of exhibiting lower 25(OH)D values. Substantial details on worldwide patterns of vitamin D status at the population level and within key subgroups are needed to inform public health policy development to reduce risk for potential health consequences of an inadequate vitamin D statu
